Mi dispiace Dario. Il "medial tibial stress syndrome" (non so l'esatto termine in italiano) è uno dei più comuni infortuni dei ultra runner.
(MSKI = Muscoloskeletal Injury)
"MSKI are observed in 18% of multi-stage events (0.7-1.8 injuries/runner and 7.2 injuries/1000 h). Most MSKI involve the lower leg (35.0%), ankle (16.8%), knee (13.1%) and foot (12.6%), with main diagnosis of medial tibial stress syndrome (30.1%) and patella femoral pain syndrome (PFPS; 7.2%)." (Musculoskeletal Injuries in Ultra-Endurance Running: A Scoping Review - Scheer & Krabak 2021).
Dato questa alta incidenza mi sono chiesto se ci sono dei trattamenti o misure di prevenzione.
I risultati sono molto deludenti:-
"Prevention methods studied were shock-absorbent insoles, foam heel pads, Achilles tendon stretching, footwear, and graduated running programs. No statistically significant results were noted for any of the prevention methods." (Medial tibial stress syndrome: evidence-based prevention - D Craig, 2008)

"Our review yielded little objective evidence to support widespread use of any existing interventions to prevent shin splints. The most encouraging evidence for effective prevention of shin splints involves the use of shock-absorbing insoles. However, serious flaws in study design and implementation constrain the work in this field thus far." Quindi non c'e molto possibilità di intervenire a priori. Si può provare con solette, con diversi tipi di stretching, con scarpe giuste (credo che questo è una questione molto individuale - ognuno avrà una soluzione diversa).
Che trattamenti esistono per trattare il problema post infortunio? Sembra che il riposo funziona quanto qualsiasi altro trattamento. Personalmente in passato ho utilizzato auto massaggi con creme anti infiammatoria ma non so dire se ha accelerato il guarigione.

Those that have been performed rendered no significant findings, leading researchers to conclude that rest is equal to or better than other treatment options." Current developments concerning medial tibial stress syndrome - D Craig, 2009.
Quale sono le cause?
"The primary factors that appeared to put a physically active individual at risk for MTSS were increased BMI, increased navicular drop, greater ankle plantar-flexion ROM, and greater hip external-rotation ROM." Praticamente o sei più a rischio di MTSS secondo le tue caratteristiche morfologiche o non lo sei.
Volevo trovare delle soluzioni da condividere ma è stato come un schiaffo in faccia. Mi dispiace.
